Workout Release
I, the undersigned, acknowledge and expressly understand that the participation in The Body Warehouse Kettlebell Workshops, Kickboxing Aerobics, Fighting Fit Boot Camp, Personal or Self –Defense Training, or any class is done at the participant’s sole risk. You understand that these activities will challenge your physical and mental limits and carry the potential of serious injury and death.
It is always recommended that each person should seek medical approval prior to participating in this, or any exercise program.
It is also understood that this activity is physical in nature. Given the physical activity and duration of the activity individuals may experience a number of reactions as a direct result of said participation. If they feel pain or other unusual symptoms, related to your training or otherwise, that the undersigned should contact a physician.
It is expressly understood and agreed that California Elite Sports Center, World Champion Karate, or any Facility utilized by The Body Warehouse, Paul A. Daniels, his representatives, agents assignees, or instructors acting under his expressed direction, shall forever be released from liability with agreement not to sue, and forever be held harmless in the event of any loss or theft of goods/property, injury, and/or death to any participant(s). By executing this instrument, the undersigned assumes all risk and responsibilities for the named participant(s).
I understand that any photos or videos taken during said activities may be used for promotional purposes only.
